Text Registration Form step-by-step guide
If you have met the necessary requirements for text message registration, you can go ahead and register. Below is a step-by-step guide on our registration form and what you'll need to fill it out completely and accurately!
Page 1: Registration
Legal Business Name
You will need to have a tax form handy when going through registration. The way that The Campaign Registry validates your registration is through matching your legal business name and tax EIN with your registration.
*If you are a sole proprietor, you will just need to provide your business name and we will prompt a text verification for business identity.
Registered Business Address
Just like with legal business name, this address will need to match what's on your tax documents.
Here, you will include the street address, city, state/province, and zip/postal code.
Business Type
Choose what best describes your business from this drop down menu
Tax EIN *
The Tax EIN can be found on box b of your W2. It's a nine digit numbers, for example 12-3456789.
If your EIN is your social security number, you will need to register as a sole proprietor.
*If you are a sole proprietor, you will not need to enter your EIN— instead, you will need to provide your mobile number in order to receive a text message to verify your identity.
Business Industry
Just like with Business Type, choose what best describes your business from this drop down menu.
Website URL
Enter your company's website URL here.

*Websites that are inactive, domain parking spaces, social media URLs, or lacking compliant privacy policy and opt-in information will lead to an automatic registration failure.
First Name
Last Name
We highly recommend that the first and last name belong to the business owner, or a business owner, or whoever is on the tax documents.
Email Address
Enter your primary business email address— we recommend using the email address that you use to log in to Levitate with.
Phone Number
This can be your office landline number, or main office number. You do not need to enter the number that you are texting with, necessarily.
Position
Choose the position of the registrant (whoever's name is going on this form) from the drop down menu.
Page 2: Texting Consent Information
Before submitting your registration we'll need to gather some information about how you are obtaining texting consent from your customers.
Are you a direct lender?
Check 'Yes' if you are in the Mortgage industry or offer any type of financing on products you sell.
*If you are in the Mortgage industry or offer any type of financing on products you sell and fail to check 'Yes' you will automatically fail. Checking yes will not impact your registration negatively, it is just a matter of disclosure.
Are you going to include phone number(s) in the text?
For the most part, the text message sent through Levitate are either one-on-one simple messages or are mass texts that look like one-on-one simple messages. If you plan on sending things like links or phone numbers, that typically raise red flags for carriers when looking for spam message, you must disclose here.
If you check 'Yes' you must provide a sample text that would include a phone number. It could look something like this:
"Hi, it is Carson at Levitate. Please give me a call back at 919-123-4567 to get an updated insurance quote."
Including numbers in texts will not keep you from being approved for registration, it just needs to be disclosed if you plan on including them.
Provide the URL of your privacy policy
To register, your website must have a compliant privacy policy. If you've gotten this far on the form, your website and privacy policy have most likely been vetted by someone at Levitate and given the green light. If this isn't the case, please reach out to your Success Specialist and have them take a look at your website and privacy policy.
If you submit this form and do not have a privacy policy, you will be failed automatically.

Why do I need a privacy policy to text?
Are you collecting phone numbers on your website?
You must check 'Yes' or 'No' here. Your choice must be factual— if you check 'No' when you do in fact collect numbers on your site, you will be failed automatically.
'Yes'
If you check 'Yes' you will need to provide the URL to the page on the website where you are collecting phone numbers. This is where your website is checked for opt-in language. If there is a field on your website that collects phone numbers, there must be an accompanying opt-in form to give the client, contact, visitor, information about consent and unsubscribing from text messages.
You are also required to upload a screenshot of the form with the opt-in language here.
'No'
If you check 'No' you'll need to provide your method of collecting phone numbers.
- Offline Form
- If you are collecting numbers on an offline form, i.e. a sign-up paper or something of the like, you must include a screenshot or file that shows that you have the proper opt-in language on the form for clients to read before providing phone numbers. This language must include stipulations about opting out, consent, and number usage.
- Offline Verbal
- If you are verbally asking clients for their phone numbers, you must type out a 'script' i.e. provide how you're asking for numbers from clients in the provided text box. It must include stipulations about opting out, consent, and number usage.
We have sample language available that you can edit to make work for online forms, offline forms, and offline verbal collection. If you submit and do not enter this language, you will fail automatically.
